Outlier Health and Wellness

Outlier Health and Wellness is a House call and telehealth medical practice that provides competent medical care in the patient's homes as well as virtually. We specialize in providing primary care for the elderly and chronic care homebound patients. We provide transitional care visits for those patients recently discharged from a hospital or facility such as a rehab or nursing home. This service allows patients to have a doctor care for them at home until they are stable enough for outpatient visits with their primary physician. Virtual/telehealth visits are available for patients that are stable and need follow-up on a chronic condition. These visits are also used for simple problem visits and our medical marijuana visits. Our certified providers can provide prescriptions and certifications for medical marijuana to eligible patients in New York State.
